To request a membership change or cancellation, complete the form below. A valid phone number is required. Management will review your signed membership agreement and call you within 2 business days after reviewing your membership details.
Submitting this form does not immediately terminate your membership, stop a scheduled payment or eliminate an existing contractual balance.
Cancellation After Your Contracted Term:
After the original contracted term has been completed, your membership continues on a month-to-month basis.
Month-to-month memberships may be canceled with 30 days notice. Any payment scheduled during that notice period may still be processed, and you will continue having gym access through the applicable paid period.
Early Contract Cancellation
If you request cancellation before the end of your contracted term, the remaining contract balance becomes due in full, along with any outstanding payments or applicable fees, as stated in your signed membership agreement.
Limited exceptions will be considered for:
Moving more than 30 miles away
A severe injury, illness or disability that prevents use of the facility
Other circumstances protected by applicable law
Proof of a new address, medical documentation or other reasonable verification will be required. Approval is subject to the membership agreement and applicable law.
Changing Your Membership:
You may request a change to your membership plan with 30 days notice.
If your next payment is scheduled within the 30-day notice period, one additional payment will be processed at your current rate before the change takes effect.
Changing plans does not automatically shorten or eliminate an existing contractual obligation. Management will review your agreement and explain when the requested change can take effect.
What Happens Next
After submitting the form:
Management will review your membership type, contract dates and account balance.
You will receive a phone call within 2 business days.
We will confirm your request, notice period, final payment date and any remaining contract balance.
Written confirmation will be provided once the request has been processed.
